Type of Insurance Product

I will make sure to call the carrier myself and confirm:

  • Correct upline designation – FMO: TKO Financial Network (The Kaizen Organization, LLC)
  • Compensation level – compensation grid by product (not all products pay the same)
  • Advancement schedule (if applicable)
  • Address on file
  • Email address on file for underwriting communication
  • Phone number on file

I further acknowledge that if I am transferring an existing contract, the carrier may require me to remain at my previous compensation level before honoring the compensation level offered through TKO Financial Network. I understand this time period may be six (6) to twelve (12) months, and I agree that it is my responsibility to notify the TKO contracting team when a compensation increase request should be submitted.

This form will be emailed to me so I can double-check what I need to ask when I call the carrier.
